Description
Hennepin County Public Health Clinical Services is seeking a Case Management Assistant to join the newly created Access team for the Mental Health Center (MHC). This position will provide case management services to clients seeking mental health services, including screening and assessments.

Location and hours:
This position is hybrid and will be performed both on-site at the Mental Health Center at 2215 East Lake Street, Minneapolis, MN 55407, and remote as job duties require. Staff will be required to come on-site for meetings, trainings, or for any other reasons that their supervisor or leadership may deem necessary (this may include onboarding and new employee training). Supervisors will provide as much advance notice as practicable. Work hours are primarily Monday through Friday from 8 a.m. to 5 p.m. Some evening hours are also possible. No weekend, overnight or holiday shifts are required. While this position is designated as hybrid, based on current requirements hires must reside in Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will:
Conduct screenings and assessments for clients seeking mental health services.
Evaluate and process all internal and external referrals to the MHC.
Establish and oversee the intake process, including a new client intake phone line.
Conduct eligibility screening and registration in Epic electronic health records system for new or potential clients.
Work with care coordinators to establish care coordination methods for social services.
Need to have:
One of the following:
Two years or more of responsible, paid social service experience providing direct client service.
A two-year human services certificate.
Two years or more of college coursework in the behavioral sciences.
Nice to have:
Experience:
Working and connecting with clients of all ages from culturally diverse backgrounds and marginalized communities.
Working with health insurance.
Using the Epic electronic health records system.
Knowledge of community resources and social services available to clients.
Ability to:
Work in a fast-paced environment, multi-task, and manage time.
Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
Establish and maintain professional working relationships with clients, staff, and the public.
