The Behavioral Health service area of Human Services and Public Health (HSPH) is seeking a Case Management Assistant to work in the Behavioral Health Center. The walk-in center serves Hennepin County residents 18+ living with substance use disorders and/or mental health conditions. We offer a voluntary social service led response to meaningfully connect residents to services and supports that match their needs. The Behavioral Health Center is an alternative to unnecessary jail or hospital use for Hennepin County residents who have needs related to mental health or substance use.
In this position, you will:
Meet with residents to assess their needs, gather basic information, and explain services offered at the Behavioral Health Center.
Connect residents to interdisciplinary team on site, coordinate referrals to existing care team, and refer to community resources as needed.
Prepare and maintain case records, complete file clearances & documentation, open intake and assessment workgroups in an online database.
Track inventory of client supplies, snacks, and personal protective equipment and assist in creation of tools for efficient workflow.
Work with various diverse communities who may have mental health, substance use disorders, and trauma.
Need to have:
One of the following:
Two years or more of responsible, paid social service experience providing direct client service.
A two-year human services certificate.
Two years or more of college coursework in the behavioral sciences.
Nice to have:
Experience:
Serving individuals living with substance use disorder and/or a mental health condition.
Interacting with people from diverse backgrounds including efforts towards disparity reduction.
Working in a health care setting, such as a clinic, hospital, or walk-in center.
Working in a fast-paced environment requiring multitasking and strong time management skills.
Knowledge of:
County and community resources for mental health, substance use and substance use disorders, social services, and basic needs.
County and state record systems.
Ability to:
Work with unpredictable and challenging situations calling for sensitivity, flexibility, resourcefulness, and independent judgment.
Organize information and assist with maintaining an efficient daily workflow.
Work well in a group setting; be accountable to the team.
