The Office of Multi-Cultural Services within the Initial Contact and Assessment area of the Health and Human Services department is seeking a bilingual Case Management Assistant who is fluent in Hmong or Vietnamese, to be part of a team that assists Hennepin County residents. This position will focus on working with recent immigrants and limited English proficiency individuals (LEP) in accessing services that support their needs to allow them to live healthy, safe, stable, and independent lives. This position will provide direct service to LEP residents 50% of the time and will be responsible for conducting community outreach the other 50% of the time.
Location and hours:
This position is hybrid and requires working on-site at the South Minneapolis Human Services Center, 2215 East Lake Street Minneapolis, MN 55407 or in the community involving direct contact with residents. Work hours will be Monday through Friday, 8am to 5pm requiring some flexibility in scheduling with occasional weekend and weekday evening shifts (not to exceed 40 hours per week).
This position will be split 50/50 in serving the Hmong or Vietnamese communities as noted and in conducting community outreach targeting all persons of Limited English Proficiency, as well as new immigrants residing in Hennepin County.
In this position, you will:
Seek out, interview, and assist people in need of agency services with an emphasis on the Vietnamese and/or Hmong community in Hennepin County.
Seek out, participate, and attend community events to build relationships with county residents and community organizations.
Serve as a cultural broker between county staff and members of the community.
Visit individuals and families in their homes (if necessary) to explain supportive services and resources available to help them meet their needs.
Interact with schools, medical personnel, and other agencies to facilitate communication between them and our mutual clients.
Inform county residents about available resources and assist them in completing applications and making appointments.
Co-locate in community-based organizations.
Work closely with community engagement coordinators in pushing engagement work across our area.
Network, collaborate, and partner with our internal and external partners on community engagement work.
Need to have:
One of the following:
Two years or more of responsible, paid social service experience providing direct client service.
A two-year human services certificate.
Two years or more of college coursework in the behavioral sciences.
Access to reliable transportation to travel to service sites and community events within Hennepin County.

Experience:
Working directly with individuals with limited English proficiency or newly arrived immigrants.
Performing face-to-face outreach within diverse communities.
Knowledge of community resources and social services available to clients.
Effective people skills and the ability to work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
Ability to:
Communicate fluently in Hmong or Vietnamese in addition to English, both verbally and in writing.
Establish and maintain professional working relationships with clients, staff, and the public.
