NorthPoint Health & Wellness Center seeks a Pediatric Nurse Practitioner to provide the full spectrum of ambulatory pediatric and adolescent health care in a community clinic. Our practice is a diverse, urban, community health center in North Minneapolis whose mission is “Partnering to create a healthier community.” This position will provide medical services at NorthPoint as well as some time at our Northside Teen Clinic and school-based clinic.
Location and hours:
This position is located at NorthPoint Health and Wellness Center at 2220 Plymouth Ave N, Minneapolis, MN. Hours for this position are 20-40 hours a week, Monday through Friday. Occasional coverage for evening and Saturday clinic. Specific hours will be determined with supervisor.
This position is subject to the federal Centers for Medicare and Medicaid Services (CMS) Interim Final Rule on COVID-19 Health Care Staff Vaccination. As such, it requires the job offer to be conditional based on the candidate being fully vaccinated for COVID-19. “Fully vaccinated” means 14 days after receiving the second dose in a two-dose vaccination series approved by the U.S. Food and Drug Administration (FDA) on a full or emergency-use basis, or 14 days after receiving a single-dose vaccine approved by the FDA on a full or emergency-use basis. After a conditional job offer is made, the candidate must show proof of full vaccination prior to their start date. A COVID-19 vaccination card from the Center for Disease Control and Prevention (CDC), a vaccination record from your doctor’s office or a personal digital vaccination record constitutes sufficient “proof.” The candidate must also attest they are vaccinated by completing a form on the first day of employment. In this position, you will:
Obtain health history, perform physical examinations and conduct preventive screenings to provide the full spectrum of primary care services for youth
Evaluate, diagnose and treat acute and chronic illness and disease.
Make appropriate referrals to other health professionals and community agencies
Participate in NorthPoint's quality management program.
Utilize electronic health records
Participate in the design, implementation, and evaluation of care programs based on the needs of our population
Triage those who require physician supervision;
Assist with our Reach Out and Read Program
Partner with patients and families to optimize their health and wellness.
Need to have:
Graduated from an acredited school of nursing
CPR certification or ability to obtain within 3 months of employment
One of the following
Possession of a valid license as a Registered Nurse issued by the State of Minnesota and a National Certification as a Nurse Practitioner (CNP)
Possession of a valid license as an Advanced Practice Registered Nurse issues by the State of Minnesota.
Nice to have:
One or more years of experience as a Pediatric Nurse Practitioner with a community health clinic or family practice environment
Experience working with interpreters
Ability to respect, appreciate and promote teamwork among diverse co-workers and patients in regard to race, age, gender, disability, values and lifestyle perspectives or interests
Candidates with fluency in another language in addition to English are encouraged to apply. Bilingual candidates who pass a language assessment may qualify for additional bilingual pay and duties.
